A few years ago, spending 4 months bouncing around between gyms would have frustrated me 😤 It’s almost impossible to progressively overload exercises when you’re constantly changing machines. Even if the “weight” is the same, there’s a myriad of different factors that will affect the load placed on the target muscles at different contractile lengths.

These days I’m just a different type of geek 🤓 I’m obsessed with admiring machines that are designed well, and the challenge of using ones that aren’t 🧩 I embrace the puzzle of optimising a piece of equipment and how the more experience I build doing so, the better rehearsed I am to help answer questions from coaches because I’ve done the thinking in advance 🤔

I’ve also learnt that you rarely come up with a great idea first time round. Provided you’re able to keep a mindset of what you currently think not being “the best” and there always being a slightly better solution, the more times you’ve been around the block, questioned your previous thoughts and adjusted the accordingly, the more refined your current thought processes will be 🧐

I train to force strong contractions, and I’m pretty sure that if I push myself hard enough I’m not going to get much weaker just because I’m not being a slave to my logbook 📋 Don’t get me wrong - I’m all for tracking and continued improvement in the pursuit of measured progress, and it has its time and place.

For me though, to sustain a passion for being in the gym for a lifetime, you have to follow what currently makes being in that environment as stimulating and fulfilling as possible 😃 In the past it’s been overcoming injuries, optimising for sports performance or just chasing numbers. Currently it’s growing my experience and challenging my critical thinking 🔣

Kinda cool how fun a place even the most alien and poorly equipped gym can become, once you’re equipped with the knowledge to take getting “creative” in the gym away from coming up with what is essentially resisted choreography 🕺🏻 and towards playing with The Exercise Continuum & The Exercise Equation (two RTS trademarks) ™️

【物理治療師:運動防護、居家復健都是違法?】

「民眾受惠於物理治療,但不能說我在做物理治療」
「物理治療師跟選手出國比賽,治療、放鬆、運動防護,都是違法的」

昨天是世界物理治療日,「物理治療師法」的議題再次浮現,因此想寫篇文章,討論目前物理治療師遇到的困境,能透過什麼樣的方式改善。

🔸 物理治療師在做違法的事?
物理治療師法,其中一條如下:
「物理治療師執行業務,應依醫師開具之診斷、照會或醫囑為之」
.
而物理治療師的業務包括:檢查、評估、徒手治療、運動治療、儀器治療、或任何經認定為物理治療的業務。
.
凡未經醫師看過、診斷過,物理治療師「直接地對他人做什麼,都是違法的」,例如跟隨國手出國比賽、需做居家復健的患者、特教機構的學童等。
.
🔸「先給醫師看」的用意
雖然聽起來不合理,但我認為法規的目的在於「讓醫師先排除重大、嚴重疾病」,釐清任何危及生命的可能,同時醫師也是對整體醫學最暸解的人,也扮演一個統合、領導的角色。

但在現實上遇到的問題卻是:
.
✔ 有些場域、情況,當下是沒有醫師的。例如隨選手出國比賽,在場上受傷第一時間還是需由物理治療師、運動防護師處理。
.
✔ 再者,常常醫師會是「決定」病患需不需要物理治療的那個人。但許多醫療人員對彼此的專業是不熟悉的,「我認為的不需要,不見得他就真的不需要」。例如「動完骨科手術就可以回家了不用復健」、「膝蓋痛就一輩子不要在運動或重訓了」。因此,若醫師能扮演整合者、協調專業之間的合作,這是理想的。

🔸 國術館、健身房可以,但治療師不可以?
.
「在健身房,有時教練會帶有慢性病、受傷過的學生,進行訓練。在國術館、民俗療法,師傅可以直接幫病患做整復、療傷。但上面說的這些事情,物理治療師都不能直接做。」

隨著人口老化、生活型態的改變,越來越多有腰痠背痛的文明病,或是有著三高、糖尿病等的人。對於現今的人來說,運動不只是緩解痠痛、延緩退化,更是促進健康的方式,但時常這類的人也不知道該怎麼動才是安全、有效的。而「運動治療」,以運動訓練的方式來作為「處方或治療患者」是每個物理治療師必學的專長。「徒手治療」則是以治療師的雙手直接進行治療,例如脊椎鬆動術、軟組織放鬆、沾黏破壞等。

若是以病患或民眾的最高福祉,我認為如健身房、國術館等地方,也應有合作的醫師能適時轉介,來篩檢學生跟患者的身體狀況、沒有遺漏掉的潛在疾病,避免在運動訓練或療程的過程中發生問題。

🔸 修法過了,治療師準備好了嗎?
雖然在現行的法規下,一般民眾、病患要接觸到物理治療師,過程較為繁瑣,皆需透過醫師轉介的程序。但當修法過了,各個物理治療師有沒有「獨立處理」病患的能力,是現在就要努力跟學習的,包括:
.
*對危及生命、嚴重疾病有無警覺性
*知道自己無法處理時,會不會轉介給醫師或其他專業
*治療師沒有像醫師的養成系統,如住院醫師、研究醫師、主治醫師等
*需更多的醫學學科:藥物學、影像學、營養學、內科、外科學等
*若想往運動產業,需額外學:肌力與體能、訓練法、運動營養等

🔸 修法不過,治療師能做什麼?
就我個人而言,因為有信任的醫師、教練,彼此能互相合作,也知道彼此專業的好,所以要請醫師開立診斷證明,或者讓病患或學生先看過醫生,這件事情對我來說相對容易,也能合情合理地進行物理治療。但當跟選手出國比賽、訓練時,我也自認問心無愧,需要做任何治療、放鬆、處置,我會馬上做,因為一切以病患或選手的最高福祉為目的。

其實這個法規讓我最困擾的是,「許多學生或病患是受惠於物理治療,但在特定的場域、情況下,我不能說我做的是物理治療」,訓練、放鬆、運動、調整身體等,都變成常見的代名詞。
.
現行法規要改變,非常不容易,因為牽涉著不只是病患的權益,還有許多政治、健保、利益等相關問題。但我認為可以先從「專業合作」開始,當專業間更認識彼此,更了解能帶來的幫忙及好處,病患能得到的絕對更多,勝過自己單打獨鬥。修法不是為了證明誰比誰厲害,或誰要吃下這塊市場,而是讓更多人能得到幫助,從疼痛、失能、絕望中解脫。

Consider the following:
Based on your genetic results, you have an even split between power, endurance and strength. You could benefit from training evenly across these training types.
For muscle development use high volumes at low intensities such as sets of 15-20 reps with 60 seconds rest completed 3 times. Engage in steady state training to develop fitness, an example of this is continuous cardiovascular exercise such as cycling or running for 30 minutes or more.
About Optimal Training Type
Evidence shows that how we respond to exercise is influenced by genetics. It won't be the whole picture alone, but it will be a part of the picture. Variations in our genetic profile can help explain why some people see a better response to different types of exercise. Following a genetically-guided exercise program has shown up to three fold improvements in training compared to genetically mismatched training.
Remember
This is a look at how your genetic profile might affect the rate at which you respond to different intensities of exercise, this should not be used to change your goal, but may help better personalise your most effective route to achieving that goal.
