【職業醫學】～ 噪音對腹中胎兒的影響
先說結論，因為噪音對孕程中嬰兒的影響，變因太多，目前無法準確預估；所以在職場噪音區有懷孕的婦女，我一定想辦法「調離」噪音作業區。
才不會到時候胎兒聽力受損，不知道要算誰的？
〖常見職場中的問題〗
Q：懷孕三個月前，不敢讓別人知道，所以一直在噪音作業區，面談時已經14-15週了，這樣會不會有問題？
A：基本上，我是希望職場中婦女一知道自己懷孕就要告知公司職護或是環安衛人員，以進行作業現場危害評估。
不過，嬰兒的耳朵大多在懷孕的第20週左右發育，在第24週左右開始對聲音做出反應。所以，學理上，你先不用太擔心囉。

Q：怎樣的噪音對胎兒影響較大呢？
A：依據美國NIOSH的建議，孕婦一定要避免長時間暴露在115分貝以上的噪音環境。
再來，低頻噪音的穿透力比高頻噪音強，根據文獻，聲音進到子宮內衰減的範圍從低頻約-20分貝，到高頻約-70分貝。

另外，也有文獻提到，超過500Hz(赫茲)的噪音，進到子宮內約衰減40~50分貝。

A：當然可以，但建議不要買搖滾區的票，一方面是聲音分貝會隨距離平方呈反比，一方面是搖滾區可能太HIGH，造成推擠。

➥【重點摘要】：本文為國際婦產科聯盟(FIGO)針對專業醫護人員提出孕婦及產後與COVID-19相關的過渡指引(interim guidance)。其內容涵蓋層面非常廣，包括：診斷方式、懷孕期間接受胸部X光或電腦斷層檢查注意事項、疑似或確診COVID-19的孕婦治療選擇、門診產前檢查相關事項、產婦的評估及分流、疑似或確診產婦的處理原則、產後護理及新生兒照護、心理及精神支持、污染物品的處理方式等。
由於懷孕期間免疫及心肺功能的變化，孕婦比一般人更容易發生呼吸道病毒感染，甚至進展為重症。過去SARS-CoV及MERS-CoV都是導致孕婦感染後出現嚴重併發症的原因，包括：需要插管使用呼吸器治療、轉入加護病房、腎衰竭、死亡。以SARS-CoV為例，孕婦感染後的死亡率高達25%。但到目前為止，還沒有COVID-19在孕婦發生率或是嚴重度增加的報告。在所收集的56個於第二或第三孕期感染COVID-19的病例分析發現，最常見的症狀是發燒、咳嗽；2/3病人出現淋巴球減少、CRP上升；83%病人胸部電腦斷層呈現多處毛玻璃樣變化。
目前尚未證實懷孕末期感染COVID-19會發生母嬰間垂直傳染，也還沒有足夠的證據指出哺餵母乳的風險或是母嬰隔離的必要性。如果感染COVID-19的母親無症狀或症狀輕微，仍然可以考慮在醫護人員的指導下餵母乳及母嬰同室。
簡言之，考量除了孕婦本身以外，COVID-19感染同時也可能影響胎兒甚至將來新生兒的健康。因此，對孕婦需要格外注意疾病的預防、診斷及治療。(「財團法人國家衛生研究院」齊嘉鈺醫師整理)
