Since the first day I got to know him, he’s always told me that his goal is to be the first Malaysian to go to Olympia. It felt like a tough journey and sometimes almost impossible. We had to travel far for every show as there aren’t as many shows in Asia compared to Europe or the US, and honestly it would cost a lot every time.
.
Since covid happened, he has not competed in 2 years as we can’t travel out. Planning this trip and getting all the approvals took us more than 2 months but he was determined that he wanted to go for it. And so we did, one step at a time, not knowing what to expect.
.
Winning this competition didn’t come without its own hurdles. Literally an hour before the competition, he had a really bad back spasm and he could not even walk. He was lying down on the floor and the organisers were so concern they wanted to call the ambulance and take him to the hospital. Of course, he refused. But the pain was getting worse and there’s no way he could perform on stage. I mean he could not even stand straight and flex his abs. Nothing was showing. 😑

Somehow, he met 3 of his fans while going to the toilet and they rushed to the pharmacy to get some pain killers. Thank god the show was also delayed by 2 hours. That gave him time to slowly recover and somehow he managed to perform his best.

Sometimes, when things are meant to be and when you put the effort and visualise what you want, the universe has its way to challenge you but also align everything for you. One thing we also say to each other is do your best in everything you set your mind to, and remember to have fun! Otherwise, what’s the point?

【吊單槓、腰椎牽引,拉開脊椎?】

「你這個是腰椎狹窄、壓迫神經,要去拉腰」
「回去吊單槓或倒吊機,可以把椎間盤擠回去」
.
腰椎牽引真的能「拉開脊椎」,其實是我一直抱持疑惑的,有些下背痛的病人做完的效果很好、也有人做完更痛。如果說牽引根本拉不開骨頭,那背後的原理、對身體的改變是什麼?
.
🔸 什麼是拉腰/腰椎牽引(Lumbar traction)?
復健科、骨科診所裡,躺著一排排、都綁著腰帶的人,這樣的場景應該蠻常見的。它不是刑具,是俗稱的「拉腰」或「腰椎牽引」,一種物理治療儀器,透過外力將腰部做一個拉開、牽引的動作。

🔸 腰椎牽引能把「脊椎拉開」?
造成腰痛的原因真的數不清,我們也常看到X光片上的脊椎狹窄,就認為是壓迫到神經、關節炎。因此,我們也推斷「增加脊椎間的距離,能減少壓迫、擠壓」。
.
然而,整理過去的研究,腰椎牽引設定的拉力,要至少體重的一半或一倍以上,才能拉開脊椎間2 mm的距離。牽引時,光是要抵消掉「床本身的摩擦力」以及「下半身的體重」就已經需超過體重的一半了,剩下的拉力才能傳遞到病患的身體上。普遍診所設定的拉力多在「體重的1/4到1/2」,所以能不能把「脊椎拉開」或效果是不是來自「脊椎骨之間的距離增加,而減輕壓迫」,我是存疑的。

🔸 吊單槓能把「脊椎拉開」?
有許多人,當中包括醫師或治療師,會建議吊單槓,因為能「放鬆腰、把脊椎拉開」。這樣的說法我認同一半,我認為吊單槓「有可能放鬆腰」,但對部分人反而會更不舒服,主要原因有兩個:
.
✔ 腰椎處在的姿勢
被吊著時,因為重力、軟組織張力的影響,腰椎多會處在「過度前凸(hyperlordosis)」的姿勢,過度前凸會讓腰椎類似卡住、更不容易被拉開,這也是為什麼在復健科做牽引時,膝蓋會被抬高,就是要避免腰椎前凸、處在一個更好被拉開的姿勢。再來,過度前凸時,是讓腰部肌群處在較縮短、緊繃的位置,就像許多穿高跟鞋、久站的人,都容易下背緊繃跟痠痛,因為高跟鞋會使腰椎更容易前凸。

✔ 腰椎接受到的拉力
吊單槓確實用「體重」將身體拉開,但無法確定「這個拉力的比重,分佈在哪個身體部位」,相較之下,腰椎牽引機是固定腋下及腰部的位置,「較有機會」將拉力侷限在腰部。再者,就像前面所提到的,真的要將「脊椎拉開」,需要的重量非常重。

🔸 為什麼牽引、吊單槓會比較舒服?會更痛?
從過去研究及我的觀點,牽引、吊單槓的效果可能來自「伸展、放鬆周遭的肌群」,許多人下背痛的原因可能源自「軟組織、肌肉緊繃」而非「椎間盤突出、腰椎狹窄、壓迫神經」等。因此,若療效可能是來自「緩解軟組織的緊繃」而非「拉開了脊椎骨之間的距離」,但是因為減少肌肉的緊繃,而間接減少壓迫是有可能的。
.
最後要提醒,有些人是不適合拉腰的。肌肉不自主地緊繃或痙攣(spasm),有時候是種保護機制,想要讓受傷的部位維持一定的穩定性,這時強行伸展或拉開肌肉,可能會引起更劇烈的疼痛,就像是拔河一樣,我們常誤以為伸展時,身體會乖乖聽話,叫肌肉放手不要出力,讓我們能贏得比賽,但身體會緊繃多有背後的原因,而用意常是「保護身體」。

【醫師、教練為什麼要學肌動學?】
.
以下這些名詞,可以稍微檢測自己對肌動學的了解:
.
「arthrokinematics, osteokinematics, kinematics, joint movement (slide/roll/spin), creep, antagonist, length-tension curve, lumbopelvic rhythm…. 」(文章的最後還有更多名詞)
.
「醫師的養成教育裡面沒有『肌動學』,但我認為對有在處理肌肉骨骼系統、運動傷害等的科別,包括復健科、骨科等,如果能對肌動學有基礎的概念,那個醫師會變超強」
.
「教練們常說受傷要找物理治療師,可能是我們對於『動作分析』及『運動治療』的擅長,而這兩種能力最大的貢獻莫過於肌動學」
.
最近剛跟幾位朋友,進行完肌動學的讀書會,讀的是物理治療系在肌動學課程的指定參考書。其中兩位朋友,分別是疼痛科跟復健科醫師,都很認同肌動學的重要性,我也認為肌動學對「肌肉骨骼系統、運動傷害、運動訓練」等領域的專家是不可或缺的,所以寫了這篇文章給大家參考。

🔸 什麼是肌動學?
肌動學(Kinesiology),簡單來說,是「人體動作的知識(the study of human movement)』。更詳細的說法是「探討肌肉骨骼系統的結構及功能,以及肌肉骨骼系統在生物力學如何應用」的學問。
.
物理治療師,普遍是在沒有影像學檢查或動刀下,就要直接分析病人的問題、可能的診斷。因此,「所有的骨頭跟肌肉的位置、每個肌肉會產生什麼動作、關節是如何移動、會影響到什麼軟組織、由哪個神經所支配、會與其他肌肉怎麼合作等」,都是必須從肌動學,習得的知識。
.
🔸 物理治療師的腦袋在想什麼?
肌動學就是讓我們腦中有一個畫面,想像「病人受傷時,身體裡面的肌肉骨骼系統發生了什麼事情。」
.
心裡想的可能是:
.
「他深蹲時髖關節會痛,可能是femoral head 做的inferior glide limited,而abnormal arthrokinematics來自iliopsoas的spasm,spasm/guarding可能是在lumbopelvic stability不足所導致」。
.
最後,這個深蹲時,髖關節會痛的病人是需要做腰椎穩定的訓練。
.
也可能是:
.
「他深蹲時髖關節會痛,是因為adductors longus 需要很大的 eccentric control,而eccentric contraction產生的張力又大於concentric時,因此在下降時會痛,上升的過程不會痛,而adductors 的 tendon反覆拉扯造成pubic symphysis的irritation」
.
同樣都是深蹲時髖關節會痛,這個病人則可能需要先由醫師做些消炎、止痛的處置,再配合其他離心訓練。
.
🔸 學肌動學對不同角色的好處?
.
✔ 對於醫療人員來說(治療運動傷害、肌肉骨骼疾病的),可以了解:
*同樣是髕骨肌腱炎,為什麼復健訓練的動作不同?
*怎麼透過伸展、關節鬆動術,來牽動五十肩沾黏的位置?
*健康跟受傷狀態下,肌肉的使用上會產生什麼改變?
*棘上肌肌腱在什麼情況下受力最大?
.
舉例來說,骨科醫師針對十字韌帶斷裂,可能會進行手術來重建韌帶,但十字韌帶之所以受傷可能來自關節穩定性不足、肌力等,要分析或找出受傷的原因,肌動學就是那個橋樑。
.
「注射、手術、藥物等,大多是在處理受傷造成的結果,但要找到受傷的原因,必須從動作的分析、了解身體怎麼產生動作、軟組織跟骨頭是怎麼交互作用等」
.
✔ 對於教練、訓練師來說:
*肌肉擺在什麼長度下力量最大?
*髕骨在膝蓋彎曲多少度,接觸面積最小?
*訓練臀中肌的蚌殼訓練,為什麼讓髖擺在45度的位置?
*健康跟受傷狀態下,肌肉的使用上會產生什麼改變?
*針對不同運動項目,怎樣的訓練可以讓身體發揮最大的效率?
.
🔸 重點整理:
*醫療上,肌動學是分析跟評估「肌肉骨骼疾病」、「運動傷害」很重要的武器
*訓練上,肌動學是能了解各個動作的優缺點、特性、哪些能做、哪些效果好
*專業之間的差異,就是對各個領域或科目,鑽研的深度不同,例如,物理治療師的肌動學、醫師的病理學、藥師的藥理學、教練的訓練法
*開始唸肌動學吧!

Until today I'm still a little shocked. The shoulder pain that I have been having for over a decade has subsided to a point that I have stopped realizing that there is pain anymore. Of course, not a 100% yet, I only went once at the time I'm writing this.
I used to teach and practice Taekwondo when I was younger. During competitions, kicks to the head is double the points of kicks to the body. The constant blow to my head over time has caused a small part of my cervical to moved a little causing a nerve pinch on the nerve that goes from my left shoulder all the way to ring finger (4th finger) and the pain resonates around my entire left shoulder blade. This has also caused frequent severe migraines on the left side of my head. I've undergone many different treatments from east to west but nothing has given much relief. Normal physio, tit tar, acupuncture, qigong, tuina, paida, heat therapy, even special prayers, you name it.
Everyone who knows me knows that I always lean a little to my left side due to the pain. They also know I always have migraine on my left side of head. And I constantly rub my back against any edge of the walls I can find anywhere, like an animal cleaning its fur.
Yesterday, I went to So Stretch and Zaki, one of their physiotherapist assisted me. Together with the help of his teammates, we did a lot of pretty intense assisted stretches which I can never perform on my own. Some of them were quite difficult because my shoulder has been as stiff as rock for over a decade. After the session, I felt pretty sore on the shoulder area but the past pulling sensation were reduced. But, today for an entire day, I couldn't help it but to realize that a bulk of the pain is not present anymore. There is still a mild pain deep within my shoulder when I lift my shoulder toward my ear. Looking forward for my next few stretching sessions, but here is my simple thank you letter for now.
Update : Pain is mostly gone now. Just a little muscle spasm left from time to time.
