【產科評估產程進展奉為圭臬的產程圖】
當年住院醫師時期值產房班,最重要的功課就是把每個產婦的產程圖畫出來。但是近年連Friedman教授本人接受ACOG(美國婦產科醫學會)訪問時都強調,這在當年只是一個統計數據的呈現,沒想到被拿來當成產程遲滯的判斷標準,每個產婦都該有自己獨一無二的產程進展。
日前接生一個第一胎的產婦,從進入產程到寶寶出生,收縮間隔都是5至7分鐘,有時甚至長達10分鐘,所以從上午9點收住院的1指半到寶寶出生將近一天。若要依照產科教科書的建議,這樣的案例,要不就加催生藥或人工破水,要不就以產程遲滯開刀了。但這就是這個產婦與寶寶獨特的產程進展,「慢慢來」。如何靜心等待不介入,對我而言是很重要的學習案例。
這個1955年的統計數據,當時的產婦生產條件為何?那是大量使用產鉗的年代,而今被真空吸引器所取代。剖腹產率1.8%!讓人嚇到吃手手,懷疑小數點有沒有點錯,認真想想或許這才是自然該有的剖腹產率?阿萍醫師要朝這個目標邁進。催生藥物的介入13.8%不算多,但看了之後很難想像,96%的產婦用了鎮定藥物,在神智不清甚至是睡著的狀態下生孩子。而今雖有硬膜外注射的減痛分娩,不至於影響到清醒程度,但那還是一種藥物介入,對母嬰一定有影響。
Who were the women in Friedman’s Curve?
Dr. Friedman’s study focused on 500 first-time mothers who gave birth at term in 1954. Their age ranged from 13 to 42 years old, and 70% were between the age of 20-30. More than half of the women had forceps used on them during delivery (55%), and only nine people (1.8%) gave birth by Cesarean. There were 14 breech births (2.8%), four twin births (0.9%), and four stillbirths or newborn deaths. The babies ranged in weight from 4 lbs. 9 oz. to 10 lbs. 6 oz., with most babies falling into a normal weight range (5 lbs. 8 oz. to 8 lbs. 13 oz.). Pitocin was used to induce or augment labor in 69 people (13.8% “Twilight sleep” was common practice at the time, and so 117 of the women (23%) were lightly sedated, 210 (42%) were moderately sedated, and 154 (31%) were deeply sedated (sometimes “excessively” sedated) with Demerol and scopolamine—meaning that 481 (96%) of the women were sedated with drugs (Friedman 1955)

Ingredients(makes 8 buns):
for the filling:
100g navy beans
200g pumpkin
80g sweetener(I used cane sugar)
for the dough:
300g bread flour
50g pumpkin skin
120g water
5g dried yeast
1 tsp spirulina(just for color. you could skip this)
10g sweetener(I used palm sugar)
15g oil
3g salt
some pumpkin seeds for garnish on top
8 80cm/30inch strings
Instructions:
1. soak navy beans overnight then boil. add sweetener and keep simmering for 30 - 40 minutes until it gets soft and water is almost gone.
2. steam pumpkin and separate the skin and the flesh.
3. blend beans, pumpkin and sweetener with food processor.
4. put all the ingredients of the dough and knead well.
5. wait until it doubles in volume.
6. divide the dough into 8 equal pieces.
7. roll out the dough to 6 inch/15cm round shape.
8. wrap the filling with the dough.
9. loosely tighten the dough with strings.
10. wait for 20 - 30 minutes(second fermentation)
11. bake at 320F/160C for 20 minutes.
12. stick pumpkin seed on top:)

1、白インゲン豆を一晩浸水し、水を取り替えて火にかけます。甘味料を加え、豆が柔らかくなり水気がなくなるまで30〜40分ほど弱火で煮ます。
2、かぼちゃを蒸して、皮と実の部分を分けます。
3、白インゲン豆、かぼちゃの実、甘味料をフードプロセッサーで滑らかにします。
4、パン生地の全ての材料を混ぜ合わせ、よく捏ねます。
5、二倍に膨らむまで一次発酵させます。
6、生地を8等分します。
7、生地を直径15センチほどに伸ばします。
8、フィリングをパン生地で包みます。
9、たこ糸などを使用して、パン生地を真上から見て8等分されているようにゆるく縛ります。
10、20〜30分ほど二次発酵させます。
11、160℃のオーブンで20分ほど焼きます。
12、タコ糸を取り、かぼちゃの種を飾りつけます。

Chocolate Chip Melon Pan (vegan)

Using fresh melon into the bread dough makes this recipe really hard to succeed. Bread dough would be sticky after first fermentation.
if you try to make melon pan for the first time, I recommend you to try melon pan recipe without fresh melon, or reduce the amount of fresh melon.

Ingredients:
For the bread dough:
250g bread flour
150g melon
25g vegetable oil
25g cane sugar
2g salt
5g dried yeast
80g chocolate chips
For the cookie dough
140g pastry flour (All purpose flour could be substitution)
40g soy milk (or your choice of milk)
30g coconut oil (you could use any kind of oil instead)
50g cane sugar
1 tsp vanilla extract
Instructions:
1. Mix all the ingredients of cookie dough, and put it in a fridge for 30 minutes.
2. Cut melon and make it into puree with blender. add dried yeast.
3. Mix all the ingredients of bread dough and knead well. leave it at room temperature until it doubles in size.
4.Divide the both dough into 8 equal pieces. Add chocolate chips into bread dough.
5. Roll out the cookie dough to 2mm thick. Roll out the cookie dough until a size larger than the bread dough.
6. Cover the bread dough with the cookie dough. make patterns on surface.
7. Wait until it doubles in size.
8. Bake at 320F/160C for 20 minutes.

「作り方」
1、クッキー生地の全ての材料を混ぜ合わせ、冷蔵庫に入れておきます。
2、メロンをブレンダーでピューレ状にし、ドライイーストを加えます。
3、パン生地の全ての材料を混ぜ合わせ、捏ねます。常温で二倍になるまで一次発酵させます。
4、パン生地を8等分し、チョコレートチップを加えます。
5、クッキー生地を8等分し、2mm厚くらいの円形に伸ばします。
6、パン生地をクッキー生地で包み、ラティッシュカッターでメロンパンの形を作ります。
7、二倍に膨らむまで二次発酵させます。
8、160℃のオーブンで20分ほど焼きます。
