聖潔莓是一種天然植物西洋牡荊樹(Chasteberry tree,agnus castus)的果實,是歐洲常用的傳統草本。

居家護理師對於新案留置胃管管灌都有莫名恐懼,擔心自拔、擔心滑脫、擔心阻塞、擔心處方藥物不能磨粉造成可能風險...,昨晚某粉絲分享一段影片介紹用空針方便磨粉的技巧給我,我順便提醒有些藥品不宜磨粉,甚至該說「不適合管灌的藥品」要注意,不是所有醫師開回家的藥都不需檢視,領藥的時候與藥師互動過程,應該為了用藥安全多問兩句,確保正確使用與安全。網路也很方便,若對於用藥是否可磨粉、泡水、咀嚼服用還是應該與醫師藥師護理師討論過。藥到病除而非藥到命除。

鼻胃管為管子連接鼻子和胃部,鼻腸管為管子連接鼻子和小腸,另外也有胃造口管和腸造口管。管灌病人藥品須經由磨粉或溶解的步驟給予,然而並非所有藥品都可以磨粉,某些特殊劑型若磨粉將改變藥效,部分藥品與管灌飲食有交互作用。管灌病人在使用藥品時,需格外注意,先諮詢藥師或醫師,以下這些皆為不能磨粉的藥品。
緩釋劑型:緩釋劑型藥品為了讓藥品達到長效,多有特殊劑型設計,一顆錠劑或膠囊等同多次劑量。若貿然磨粉將使藥品一次大量釋放,增加藥物毒性,同時藥品因失去原本的緩釋設計而無法持續到原先設計的給藥間隔時間。
例如降血壓藥物Nifedipine (Adalat OROS)屬於特殊劑型,若 #磨粉將導致藥品大量釋放進而導致病人低血壓。#緩釋劑型藥品英文結尾多有CR、ER、LA、SR、XL、XR、OROS等字樣,#管灌病人使用長效藥品建議以同成分速效劑型藥品替代。
舌下錠:含於舌下,經由口腔黏膜吸收作用較快,例如耐絞寧錠。若藥品磨粉後,管灌將無法達到預期效果導致治療失敗。
腸溶劑型的藥品:腸溶衣錠必須到達小腸才能發揮作用,藥品設計有保護層避免被胃酸分解。例如胃藥Rabeprazole (Pariet®),藥品被磨粉後由鼻胃管投予在胃部即被分解而藥效不足。鼻胃管病人不建議服用此類藥物,鼻腸管與腸道造口病人可以使用。
含致癌毒性的藥品:含致癌性的藥物例如化療藥品、標靶藥品不建議磨粉,以避免傷害給藥的照護者。部分藥品在安全的環境下可泡水崩散後立即給予管灌病人使用。
易造成管路阻塞之藥物:分散型膠囊藥粒會塞住管子,例如抗癲癇藥Topiramate (Topamax®)。緩解便祕藥Sterculia/ frangula (Normacol®)與Psyllium hydrophilic mucilloid (Konsyl®)吸水後會膨脹凝固阻塞餵食管,以上藥物不建議管灌病人使用。
內容物為液體的軟膠囊:內容物為液體的藥物不建議磨粉,包括cyclosporine (Sandimmun Neoral®), progesterone (Utrogestan®)。
#管灌病人需注意服用藥品的劑型適當性與交互作用,請諮詢醫師與藥師以利藥品的選擇。管灌病人選擇適當藥品不但可以達到療效,也可減少副作用發生。

A really informative chat with fitness and health expert Ben Siong. For all my sisters, who have always questioned bout why they have irregular periods, why they bloat, why they have painful periods, what's the possible cause for bleeding more than two weeks here's a video that will help explain those questions!
The human body is very complicated and my dear sisters, ours is even more complicated cause we have a thing called the period. Ben is one of the few coaches who takes the time to not only understand the physiology of the body but also the mind which is incredibly crucial. And would female athletes have to then avoid highly taxing sessions/peaking periods during this time frame?

Is this accurate? If so should females generally avoid deadlifts/front squats/lower back intensive exercises during this period? Should they be left out entirely, or replaced by something else?

6 weeks out.
I've entered a different playing field as i prep for my comp. I do believe it's not impossible to get bikini stage ready, but taking the progesterone meds to balance out hormones in my body will prove to be a complication. Making it a bit more complicated for me and my coach to assess my body composition. Currently, i don't see any bad changes in my physique, hence the plan is to continue with training and nutrition plan, while seeing what happens. Staying positive for the stage!!
