It’s an English metaphorical idiom for a significant problem or controversial issue which is obviously present, but ignored or avoided as a subject for discussion. Therefore people in the room who pretend the elephant is not there, have chosen to avoid dealing with the looming issue...kind of like when someone in the family has a life-limiting illness but no one knows how to approach the subject or want to talk about it. How do we deal with such conversations? Or do we avoid, & hope that all will be forgotten?

Hospis Malaysia’s “SPEAK UP! THERE’S AN ELEPHANT IN THE ROOM” initiative hopes to encourage conversations that matter for patients diagnosed with life-limiting illnesses. By bringing up issues, unfinished business and hopes with our loved ones & healthcare providers, speaking up can be the beginning of a journey of a life well-lived despite the illness.
